Knowing what to do is rarely the problem. The problem is making it stick — through real life, difficult weeks, unpredictable symptoms, and the very human experience of getting stuck and starting over.
The Foster Health 8-Week Breakthrough is built around a simple but powerful idea: lasting health habits are not built in one session. They are built by returning to the same proven process again and again — refining, adjusting, and embedding — until the habits become part of how you actually live.
Over eight weeks you will work through that process eight times, in a deliberately small group of twelve people, guided by Eileen Patterson. Each week builds on the last. Each cycle takes you deeper. By week eight you will not just know what to do — you will have been doing it, with support, with accountability, and with a group of people who understand exactly what that has taken.
This is not a support group. It is a structured coaching programme with clear outcomes, a proven framework, and Eileen's expertise embedded in every session.

How It works
Each week is built around four interconnected elements that work together as one system — not four separate programmes running in parallel.
90-minute peer group session Once a week, the full group meets on Zoom with Eileen. Each session moves through the behaviour change cycle — revisiting your goals, releasing what is getting in the way, building on your vital health habits, and adjusting your plan as your life and circumstances change. The group dynamic is intentional and powerful. Twelve people working through the same process, guided by the same expert, creates a level of shared understanding that individual coaching cannot replicate.
Peer check-ins in triads Between sessions, participants connect in small groups of three — triads — for brief, structured peer check-ins. These are not casual chats. They are a specific accountability mechanism that keeps the work active between weekly sessions and builds the kind of honest, supportive relationships that sustain change over time.
AI-assisted coaching support Participants receive a structured prompt to use with their preferred AI system — ChatGPT or whichever tool they already use — as an on-demand coaching companion between sessions. This is not a Foster Health tool. It is a way of making evidence-based coaching support available to you at any time of day, using technology you already have access to, guided by the framework Eileen teaches in the sessions.
Align to your existing tracking system There is no new app to download and no tracker to buy. Participants use whatever personal tracking system already works for them — an Apple Watch, an Oura Ring, a HueScale, a simple notebook, or pen and paper. The programme teaches you how to use what you already have more intentionally, in service of the habits you are building.
The Vital Health Habits
The eight weeks focus on five areas of health that the evidence consistently identifies as the foundations of lasting wellbeing:
Breathe — connecting to yourself and managing stress through intentional breathing and awareness practices.
Eat — nourishing your body with sustainable, realistic nutrition habits that support your specific health situation.
Action — building regular movement into your life in ways that are appropriate, enjoyable, and genuinely maintainable.
Mindset — developing the intentional awareness that makes it possible to notice patterns, catch setbacks early, and respond rather than react.
Sleep — building the rest and recovery habits that underpin everything else.
These are not new ideas. They are the habits that every credible health framework points to — and the ones that most people struggle to build consistently without the right support structure around them.

The 8-Week Breakthrough is facilitated by Eileen Patterson — the healthcare leader who brought the first chronic disease self-management programme to Canada and spent three decades leading health improvement initiatives across Canada, the United States, and the Caribbean.
Eileen built Foster Health because she has seen, from the inside of health systems across three countries, exactly why people struggle to change. And she has been through her own reckoning with it personally.
She knows it is not about trying harder. She knows what actually helps. And she brings everything she knows to every session — with a maximum of twelve people at a time, so every participant gets real attention, not just a seat in a room.
