A comprehensive, guided support journey for people who are ready for better health using a proven health habit change approach. This is where new vital habits become the daily routine, bringing lasting change and a new sense of wellbeing. Practical activities and insights  — guided by Eileen, and strengthened by a shared group journey. A group is not a compromise; it is an advantage.

The series runs for 8 weekly sessions of 90 minutes each; participants receive the Foster Health Guide, tracking tools, practical resources, and between-session support.  Knowing what to do is rarely the problem. The problem is making it stick — each week builds on the last, bringing new information and additional support to overcome real-life challenges. You will work through a proven process with a small group of people. By week eight, you will not just know what to do — you will have been doing it, with accountability and support.

This Foster Health Breakthrough is for you if:

8-Week Breakthrough Outline

This is a guided series that narrows in on each participant’s specific goal, priority habit, and the best way to align support. Each participant creates a solid health habit action plan and receives ongoing support as they bring that plan to life in a way that works for them.  With an ongoing focus of letting go of distractions and finding the best ways to build the habits into daily routines, improvement comes faster. Using the Foster Health Action Plan and Progress Map, participants review habits weekly, identify challenges, and address issues.  New practical tips and proven approaches are shared throughout.  Between session supports include small-group chats, alignment with trackers, and personal support. Every success is celebrated, and participants are guided to identify next steps for sustained health habits.

About Eileen

The 8-Week Breakthrough is facilitated by Eileen Patterson — the healthcare leader who brought the first chronic disease self-management programme to Canada and spent three decades leading health improvement initiatives across Canada, the United States, and the Caribbean.

Eileen built Foster Health because she has seen, from the inside of health systems across three countries, exactly why people struggle to change. And she has been through her own reckoning with it personally.

She knows it is not about trying harder. She knows what actually helps. And she brings everything she knows to every session — with a maximum of twelve people at a time, so every participant gets real attention, not just a seat in a room.

Frequently Asked Questions

  • Between weekly sessions, participants connect in small groups of three for structured peer check-ins. These are brief, focused conversations designed to keep the work active between sessions and build genuine accountability relationships within the group.

  • You use whichever AI system you already have access to — ChatGPT or any other. Eileen provides a structured prompt that turns it into an on-demand coaching companion between sessions, guided by the behaviour change framework taught in the programme. Foster Health does not provide or endorse a specific AI tool.

  • None that you don't already own. Participants use whatever personal tracking system works for them — an Apple Watch, an Oura Ring, a HueScale, a notebook, or pen and paper. The programme teaches you how to use what you already have more intentionally.

  • The recording will be available in the Slack group. However this workshop is designed as a consecutive three-day experience and the value compounds across the days — attending live is strongly recommended.

  • Yes. Many participants will be managing a chronic condition. The workshop focuses on behaviour change — the habits, tools, and mindset that support whatever your medical team is already doing. It is not a clinical programme and does not replace your medical care.

  • Yes. The workshop is delivered virtually and open to participants anywhere.
